In consideration for being accepted by my sponsoring church for participation at Bethel Youth Conference, we being 21 years of age or older, do for ourselves or myself, hereby release, forever discharge, and agree to hold harmless my sponsoring church, the Bethel Ministerial Association, and Bethel Youth Conference directors and leaders thereof from any and all liability, claims, demands for personal injury, sickness or death, as well as property damage and expenses of any nature whatsoever which may be incurred by the undersigned and the child participant that occur while said child is in participation at Bethel Youth Conference. Furthermore, we or I hereby assume all risk of personal injury, sickness, death, damage and expense as a result of participation in recreation and work activities involved therein.
The undersigned further hereby does agree to hold harmless and indemnify the Bethel Ministerial Association and sponsoring church, its directors, employees, and agents for any liability sustained by said as a result of diligent, willful, or intentional acts of said participant, including expenses incurred attendant thereto.
I give permission for the Bethel Ministerial Association to use photos and/or videos of me in promotional literature, both now and in future publications, whether printed or electronic. In addition, I have read the Conference Guidelines and agree to comply. Signature indicates compliance with a criminal history check.
